Payment by bank transfer only. If you prefer to pay by cheque, please use the standard form.

- Save to your Documents area, or to Google Drive or equivalent - devices may vary

- In the Signature field, just type your full name (if you have an electronic signature, please DON'T try to use it as it won't work!)
- Save the filled-in document
- Send the completed form as an email attachment to the Membership Secretary at membershipsecretary@wollatonu3a.co.uk with the subject line "Application for Membership"
- If the above link doesn't automatically open an email, you can copy and paste it into one instead
- Please make payment by bank transfer. Our bank details are given on the form, and also further down this page
Standard form - print and post or hand in
Print the form in your usual way. Fill in and post to the address given on the form, or bring along to the next monthly meeting or coffee morning. Alternatively, you can scan the completed form and email it to the Membership Secretary at the address, and with the subject line, given in the guidance notes for the electronic form
Bank reference: please use first name and surname (plus name of bank account if different to own name)
